I sent out the letter on Sep 17 however i checked the mailbox today i didn't receive any form yet, i am so worry.......

I called the school today and they checked for me that i will receive the application form  within these few days...(I sent the form on 17 sep)

I received the form this afternoon, the deadline to return the form still on Oct 4, interview on Nov 7, 11 am, i will return the form by hand tomorrow morning...
