When I left, there are over 60 people queue up, waiting for submitting form. I saw the head teacher there.

Thank you very much! My friends got the blank form finally.

I was submitted the form yesterday, it's really terrible that there are 100 persons line up before me, and I was spend 2 hours to submitted the form. :cry:

You just need to bring along with the kid's birth cert. original & the id card of the person who has signed on the application form . Last, don't forget to bring a $50 cheque, cash are not accepted.
