I have just prepared it by myself, based on the information required by the school, when I applied a DSS school for my son.  The profile is very simple, just listed the school activities, award granted by the school and other extra-curriculum.  I also attached some scanned photos for the mentioned events and activities. I used a simple file to bind these documents together.  

Fortunately, my son has been accepted by the said school.

I have just attended some school admission seminar, the headmistress said that just don't make it too thick as they don't have time to review, they prefer to make as a summary rather than to provide them all copies.....
