"聯課活動紀錄表" 內有說明 :

Achievement in Co-curricular Activities and Service in Primary 4 to 6 聯課活動及服務表現（請由小學四年級開始填寫）
 Please list the 15 most significant achievements in the table below. These achievements can be school arrangedactivities or outside school activities in dance, drama, leadership, music, services, speech, sports, etc.
請在下列表格填寫最多 15 項於學術成績以外最主要的成就資料。填寫項目可包括校內或校外活動如舞蹈、戲劇、領袖才能、音樂、服務、朗誦、運動等。

 Please attach a photocopy of the relevant documents for verification. Order photocopies according to earliest dateto most recent. Appendix numbers should be placed in top right-hand corner, starting with CCA 01.
請附上證明文件影印本以供核實。填寫「附錄編號」欄時，請自行為附錄編碼，順序由 CCA01 開始，有關編號亦須於副本右上角列明，並順序排列呈交，以方便校方審核。

 Please bring the true copies of relevant documents for verification on the day of interview.
請於面試當日帶同證明文件正本作核實。
