我己寄了信，但根據她們的出生要求，只能報PN class.但我唔想，我希望讀K1.
唔知有冇家長知道可否兩邊一齊報呢？

can only choose either Eng or Cantonese for interview language.

Feel sorry to inform that each form is unique. The sequence no. and interview time are printed on the form.

陳雙兒你好，我申請咗st cat k1，不過多數唔收啦(一早已預咗）另外，我囡囡返緊學啦，你囡囡仲未返呀？.  我pm 咗一d問題給你（經bk).

Is there a form?  I thought we just need to submit the required documents and wait for the application form and interview slip to be sent to us.

I don't think they will accept any PN applicants for 2013/14 now, coz they strictly stated 10 to 12 Sep 2012, and the instructions for this application were removed from their website right after 12 Sep 2012.

You are right. You just need to submit the required documents and wait for the school to mail back the application form and interview slip.
