Baby that cannot gain weight.........

My friend has a 21 months baby only weighs 8 kg.  Starting from 5 months old, she just hates to drink milk and eat other things.  My friend had taken her to the hospital to take blood sample and brain scan but the doctor just cannot find out why she has difficulties on eating (has even seen physiotherapist and speech therapist).
Now she is 21 months old and only weighs 8 kg.  She was assessed to have 發展遲緩for half a yr delay and some 自閉傾向.  My friend has taken her to private training (5 days a wk and 3 hrs a day).  She has some improvement but my friend finally stopped the training after a month cos' she found that other kids from the class has more serious problems than her.   She doesn't want her daughter to learn the bad behaviour from them (acutally I don't agree with her in this point).  Now she is going to let her try the playgroup from normal kindergarten.  
I visited her last wk and she seems to have some degree of depression cos her daughter.  Her daughter still cannot say "baba mama", etc.  Just can say, "ar! ar!"  And she eats much more now but still cannot gain weight probably.
So I would like to ask if anyone of you have seen the case like that before?  My friend is so worrying about her daughter and every time when she sees other kids of her daughter's age, she just compare and wants to cry out........
