At 2nd reg. on 9/5/09, Munsang requires new students to provide a health certificate signed by a doctor. I called Munsang and the staff said the school had no standard form; I asked two clinics and they said doctors would only sign a form provided to them.

Can anyone tell which doctor near Munsang can provide such health cert. without producing a form? Thank you very much.

Hello, I just call them and the staff said just fill-in and signed by one of the parent on their attached form is OK, no need to provide a health certificate signed by a doctor.

Thank you indeed. But this really contradicts with what I was told. Anyway, I will do as you were told as it was the easiest way to do.
