you just ask the church you received baptist for 領洗紙, it's very easy.

If your child has been baptised, you should have a paper showing that.  If not, ask for it.  Otherwise, you are not a Christian, until you are baptised.

您屬那教會呢?  天主教及聖公會均有嬰孩洗禮，然後就會派領洗紙，長大後會再有按手禮，為孩子re-confirm他的信仰。
您的情況可跟堂會溝通吧! or 請牧師寫封信証明您們經常出席主日崇拜啦!
