IB PG is based on internal assessments + teachers prediction of what student might get as a final grade. IBO has guidelines to teachers so they well know how to give PG to students. There will definitely be uncertainties but IB PG is generally considered fair prediction of students' final grades.
Canadian high school students are generally not writing any public exams. Canadian universities therefore have always been using internal grades for admission purpose. Most universities ask for up to 4 years of high school grades. Students from other curriculum (including from IB, or DSE, or AL) will also have to submit 4 years of high school transcripts. Thus, IB PG would not be the only information they consider. Most of the time, the offers from canadian universities are conditional offers.
I have spoken to an admission professor of a very top engineering program of a canadian university. They are known to have very high admission threshold within Canada. He said that they have a special database keeping track of grades of students they have taken. The database has information of (1) internal grades (2) PG and final grades (for students doing IB etc) (3) grades of freshman year classes. These information will be used as reference material in the future. So they are totally aware which high schools tend to blow up their internal grades and PG.
