多謝大家！我知道過了時間，但仍想試試，“小一選校”那邊也post了，可惜未有回音。

Suggest you a method.

Retrieve a copy for the year 2009 - 2010 from the following web:

http://rcps.raimondi.edu.hk/ApplicationForm.pdf

Use correction ink to amend and make photo copy. Do it ASAP as the deadline is past.
