2021-2022 學年基階二、拓階一至拓階五插班生入學申請須知

2. 基階二、拓階一至拓階三年級全年學費現時為港幣二萬八千六百元正。
拓階四及拓階五年級全年學費現時為港幣三萬二千三百元正。
所有學費均分十期繳交。

本校設有學費減免計劃供合資格的家庭申請（包括來自綜援家庭的學生及領取學生資助辦事處財政資助的學生），所有申請資料均保密處理。有關詳細資料請參考本校網頁(http://www.logosacademy.edu.hk/tw/forms-download/#fee_remission_scheme)內之「學生資助計劃資格評估申請指引」或致電校務處，電話號碼：2337 2126（基階二、拓階一至拓階三），2337 2123（拓階四及拓階五）查詢。

3. 填寫表格時，請用藍色或黑色原子筆。

4. 填妥的申請表格，連同學生的出生證明書、香港身份證或護照影印本、最近三年的成績、獎狀或學術以外成就的證明文件影印本、現時 / 最後就讀學校之校長推薦信，於 2021年5月28日或以前，遞交以下地址：

新界將軍澳嶺光街五號（申請基階二、拓階一至拓階三）
新界將軍澳勤學裡一號（申請拓階四及拓階五）辦公時間 : 星期一至五上午九時至四時三十分
星期六上午九時至中午十二時
星期日及公眾假期休息

5. 申請者需經由校方審核文件及篩選後，會邀請合適的學生於 2021年6月5日參加筆試（半日），報名費（港幣50元正）通知書將會稍後連同筆試邀請信一併以電郵寄出，請家長到7-11便利店繳交後，帶同收據於筆試當日出示。

6. 達至評估要求的學生及家長須在 2021年6月21日至23日參加面試，時間地點容後通知。有關學校資料請參閱本校網址 http://www.logosacademy.edu.hk

7. 申請結果將在 2021年7月5日公佈，獲取錄者須在 2021年7月6日至8日到校務處辦理註冊手續。

3. The application period starts from 3 May 2021 and ends on 31 October 2021. Applicants can only apply once during the application period. (Door-knocking Round: from 1 June 2021 - 16 July 2021)

APPLICATION GUIDELINES

Eligibility to Apply

Completion of local primary 6 schooling, overseas primary education, an IBPYP course or an equivalent primary school curriculum.

Exceptional cases with specific reference to mid-year transfers will be considered on a case by case basis.

Placement is not dependent upon age or number of years of school experience.

Application Procedures

Online application is available for the application from 1st June 2021 onwards.

Application should be submitted from 1st June to 16th July 2021.

Applicants should submit the application to our school, together with the following documents:

A recent digitized photo of the applicant.

Applicant’s Hong Kong Identity Card / Birth Certificate / passport / study visa (in case of non-local applicants, a copy of student’s passport or other travel document with a copy of his/her personal details are also required.).

Primary 6 Student Record Form issued by the Education Bureau (if any).

Applicant’s school assessment reports in the past two years.

A recent piece of Chinese and English writings which have been marked and graded by the applicant’s previous school teacher.

Certificates of awards, such as art, music, sports, etc (if any).
