What can I submit?

想問你地咁多位 你地會唔會都係交電費單所以先要你地再交其他證明文件(因唔係政府部門發出), 如果一開始就交差餉單咁係唔係好D呀

You may need the following things for submission :
1) your ID card copy
2) your birth certificate copy
3) the bill under your parent's name

我估冇關係﹗可能係 RANDOM CHECK, 我都係ONLY 交個 煤氣單, TOUCH WOOD 都冇收到信﹗大家放心啦﹗聽人講年年都有 RANDOM CHECK, 可能今年比較嚴﹗因有好多人用假地址抽名校區掛﹗所以唔好擔心啦﹗

I think people concern is the address is a real, but they don't living there or hard to provide more evidence to proof even they have elect/gas/water bill notes.

If you do not have further document to provide and you insist to use that address as your real home address, they will simply come to visit you then. They call this as "家訪"。Please be aware that they may visit you more than once.

If you are using someone's address, I think you better find an excuse to change the registered address to your real address.

我早幾日已經收到信, 琴日仲有教署打電話黎同我confirm有冇收到信, 仲提我記得3/12前寄回住址証明! 我諗報假住址果d最好拿拿淋改番自己真正地址, 因為如果查到地址係假, 就算抽到, 都會被取消資格, 仲有手尾跟呀!

乜唔係當父母不在或冇能力照顧小朋友時,  才會有監護人出現?
我記得有人講過係有所謂"法定監護人", 應該唔係隨便可以叫其他人做監護人喎. 同埋如果係用監護人地址, 咁小一申請表也要由監護人申請.
不過, 你最好都係問教育局.
