Normally a formal service agreement includes two parties, details of service provided, contract price, payment method and contract duration.
You need to state clearly what sort of service needed, e.g. 3 meals plus bed, any other charges, etc.
If both parties agree the termination period is two-month, then either party should serve the other party two months notice or two months notice in lieu of payment; all unused service should be refund on a pro-rata basis. You also need to mention how to terminate, either by letter or by email?
How about jurisdiction, will you use UK law or HK law when dispute arises?
You can try to search from the internet "sample of service agreement" or visit other guardian companies' website to have a board idea of how a service agreement looks like.
I thing you need to consider, if your guardian wanted to terminate the service contract, will you be comfortable to let your son to stay there for another two months? If not, you will have to pay them two months early termination as you are the one who wants to leave early.
