transportation fee for paino teacher

It seems to be a silly question, but I really don't have any idea. If the piano teacher teaches at my home, is the transportation fee included in the monthly tutor fee? Or I should pay the exact transportation fees for a month separately? And, should i pay the monthly fee in the first lesson or in the last lesson?
