Portfolio 要求總括如下︰
(1) 不可多過 15頁 (雙面印不適合? 不包括所須提供的資料?)
(2) Portfolio 用釘書機釘起即可

報名辦法：
由2009年9月4日至9月8日，請於辦公時間內到本附屬小學索取報名表格，或由本校網址下載，或於9月4日前付回郵信封向本校索取；並請家長於9月12日至9月15日期間，親身將填妥的表格及有關文件於辦公時間內交回本校。
辦公時間：星期一至六（上午9:00至中午12:00，下午2:00至4:00）
