在10月12日到31日期間去函索取報名表
報名表會在11月6日寄發
在11月18日和19日報名
12月12日為面試日期

I still can't receive application form wor. Is it possible missing? I called the school but it's recorded message. What can I do?

you can call the school again in the office hour, so that you can sure whether you can receive it or not.
