Dear All Moms,

Kindly request one more spare CCKG Application Form.

The reason is I promised one of my friend to take the application form but I forgot at all.

Please kindly tell me if you have one extra, I am very pleased to pay extra money to have one more application form from CCKG.

May I obtain a copy of the application form too? Please kindly PM me and I'll arrange courier to pick up from you with a $20 cash cheque enclosed for the fee that you paid.
