I think I was actually the one who wrote a reply for the previous request, yet I can't find my own message, either... 
Say something like:
To Whom It May Concern:
I am the parent of ... from ..... (class). I would like to inform you that my child will no longer enroll in your school in the coming academic year. This is certainly a tough decision for us, as we know that the school has been trying its best to provide our child with a good education.
