On the date, you will need your 通知書and go to the center. Then, they will give you a form to complete. In net 41, they even provide enough chairs and tables for the parents to complete the form. The sample is just for you to practice so that you won't feel so nervous and make careless mistake. However, you should still make a ranking of the school that you want to apply beforehand.
You are not required to fill all the blanks. So don't feel obligate to fill in all the schools. In fact, I only wrote down three schools on my form last year.
