To accept the offer of a Form 1 School Place, parents/guardians of the successful applicants are requested to submit to the School IN PERSON during office hours on 18 February 2012 (9:00am to 12:00noon) or 20 February 2012 (9:00am to 5:00pm):-
1) A cheque of HK$800.00 made payable to Heep Yunn School as registration fee. Please note:-
a) Print the applicant’s name, Application Form No. , STRN and contact telephone number on the back of the cheque.
b) The registration fee is non-refundable but will be deducted from the first instalment of school fees in the coming September.
c) If you are covered by the Comprehensive Social Security Assistance (CSSA) or your family has obtained the Eligibility Certificate (EC) issued by the Student Financial Assistance Agency, upon production of the relevant certificates, the registration fee will be waived.)
2) The applicant’s Primary 6 Student Record Form (Original).
3) The duly signed Letter of Undertaking (enclosed with the offer letter) and 2 copies thereof.
Please note that the offer will be withdrawn if the above procedure cannot be completed by 5:00pm on 20 February 2012.
