dgs已出招生廣告，我有幾個疑問：~交表時是否一定要交Personal Information Form (Sports)
【冇料填】

~交 dgs嘅Portfolio Summary Form時可不可以（使唔使）交埋自己整嘅
Portfolio（定系....)

~唔夠欄位可唔可以加？

請大家指教我呢位“白癡”~

請問獎狀和證書有沒有限制從哪個年級開始填?操行及諸如品學兼優、學業獎之類應該填寫何處？校内的服務項目是否需要填？謝謝各位家長指教。

Not suggest to attached protfolio as the form is simply for first preview by DGS, i called the DGS registration office, the staff said this is not welcome.

Per DGS' advice, you can provide the appendix to extend the summary form.

That's interesting?! We sent in our portfolio last year and we saw stacks of portfolio too!! Mrs Lau really read through all the portfolio!

ha ! may be my friends and i were too honest ! When we drop the form into the collection box, we cannot see anyone submitted with a pack of portfoilo.

I guess either way is OK. I also noticed applicants only submitted portfolio with prescribed form. I would think format does not really matter too much, the substance of contents counts.

In the first round, only need to submit the following:
(1). The filled standard form download from the school web-page.
(2). The photocopy of the documents supporting the information filled in the standard form, e.g. photocopy of the related certificates.

政府規定，所有的官、津、直資中學都不可以在中學派位統一放榜(一般是7月)前用筆試篩選學生。

Student Reference No. (STRN)

You may find this number on the document 'SECONDARY SCHOOL PLACES ALLOCATION, PRIMARY 6 STUDENT RECORD FORM' from Education Bureau.

Mrs Lau and most teachers can read Chinese and English.

"份表又中又英" : It is ok

"翻譯得唔好,好似好核突" : Don't 翻譯 youself (ie.唔好翻譯)

A prize that has not got its title in English, just write it down in Chinese.
Even if your form is ending up with English/Chinese mixed, that is ok.

My only advice is: pay attention to the font size.
