You can find the sample letter from the following website

I don't have a template of the portfolio.  All you need to do is to attach the following with the cover letter:

1)  Your child's resume
2)  School reports
3)  Extra-curricular activities certificates
4)  Reference letter
5)  Pictures of interest
6)  Artwork that you child did that you think is impressive

Besides item 1, all others should either be copied or scanned in order to be good looking.  After that put them systematically in a nice binder or folder.  That should work.

各位
其實 file 內容比較自然D好!!個個都係一色一樣O既話好似唔係咁得學校留意,由其係呢度咁多媽媽都用呢封信,同一間學校可能會出現幾封,你地話D學校見到會點呢?
最好係小朋友多D參與其中,唔好咁死板,比如叫小朋友親手寫自己介紹,志願,一D手作仔,畫下畫,咁樣可以表示出這封信唔係得父母做,同埋小朋友都知道發生緊咩野事!!有助學校知道父母同小朋友O既關係!!

Agree with wwyee.  However I think some of the parents may not have the experience of doing this before, therefore gongonma can give a general guidance of how to compose it.  I made one (not copied) for my son.  It's simple but genuine.
