pm offer. requested to hand in health report, but where can I got the health report for my child? what items should be included?

Simply goto see the doctor and ask for that report, they will do it and charge around $200 ~ $300.

Need to download the health report from school website, then let the doctor check it and sign it.

Exclude the tutition fee HK$4480 x 11 months and uniform fee,are there any other fee and charges? thank you.
