Hello, if i want to apply to both campuses, should i fill in one or two application forms? If one form, which campus should i mail the form to? Many thanks!

I have checked school and each application form has registration code that we CANNOT use copy/downloaded file from others. That mean we are sure missing application.

yes, 每一張申請表都有一個編號.
