Anyone know if the health check report must be handed in upon the registeration? This is because I may go to other offer if later there will be....

Yes it has to be handed in with the $970. My doctor signed on the form provided by Think with comments. You can't use that for other schools anyways?

Yes. It is formatted in the way of think and cannot be used by other school. While the doctor will charge additional for such reports....that's bad...kind of money waste if i finally choose not going to think

how much did it cost? I have asked a clinic nearby, the nurse said at least $300, depends on the items in the report.

We're lucky - our doctor didn't charge us. He is our doctor since the day my son was born. But he did make a record on which school he has filled out for.(Frankly, I think this service is included since his usual checkup consultation + vaccination fees are soooo expensive, haha)

my doctor didn‘t charge me as well, it seems quite common for my doctor to sign such kind of report

My doctor charged me additional $150 on top of the consultation fee.

Me too for the PM class. My doctor in Sham Shui Po charges $150 for the report (including the consultant fee).
