me two....learn that the form d/l from web can't be tranasfered as there is name and no. assigned on it....how about the form get from school directly? if it's a common one and you have extra (e.g. you get one from web also), please kindly help. 九龍塘樂基係跟人名架！
ascot就張張唔同no.，除非download多咗，否則都幾難俾到人..

ascot 有4頁 (有問卷), kln tong 那份只有一頁 (沒有問卷)

唔得呀!九龍塘如果是上網dl ,就已印埋個名係份表格度，不可以重用。火炭就即場填埋出世紙號碼跟收據單號碼一齊，所以都無得俾第二個用。

火炭已在application form 上記錄出世紙號碼，九龍塘在download時要寫明小朋友姓名，兩表格均不可轉讓。各位騙子想呃神騙鬼換來一百幾十，請過主。再者，如樂基對你那麼重要，沒可能忘記她的申請日期，她們每年的申請日期也是相同的。每年也看到這些媽媽，只能說，在意的，早已有表格了。

if the situation is true, please send the supporting document to them, i'm sure they would offer you a form.

火炭樂基張2011 questionnaire 同今年嘅有些少分別, 同埋今年questionnaire都有assign number. 張Application form 有冇分別我就唔知，但係有form no，日bb嘅ID寫好左先俾我地,重有stamp左樂基校嘅籃色印章。
所以話有火炭樂基form係手嘅, 一係11月3號冇去攞form, 一係呃人! 冇其他可能！
