Parent must come in person to fill in the form. Please bring along:
(1) HK$ 30 for application form
(2) Relevant documents
i) Birth Certificate (Original & Copy). If your girl was not born in Hong
Kong, please bring the original and a copy of her Passport or Identity
Card also.
ii) Two passport size photos of the applicant.
