Which sentence is correct?

1. Peter thinks that he will fail his mathematics
examination.

2. Peter thinks that he will fail at his mathematics
examination.

3. Peter thinks that he will fail to his mathematics
examination.

it should be "fail in"

2. 不及格
She failed in the examination.
她這次考試不及格。
