請各位家長指教:

現在本人為小女填寫中一申請表,表內其中一部份要填寫最近兩年參與之課外活動.另一部份要填寫最近兩年獲得之獎項或證書.兩部份分別有八項供學生填寫,它已說明(若有需要,請加附頁).

小女的學校已為學生們處理了(資料申報表),它們分別是(校外活動及獎項)及(校內活動及獎項).

請問我應該遞交學校的(資料申報表),它給與的兩欄我是否要寫請看附頁?還是跟著中學的報名表的做法,若不夠填就另加附頁.

請各位家長指點!

HELLO, I think you have to make a personal resume as
the first page of your profile, it shown the name, address, birth date..... and you can set "extra-curricular" column to show your achievements. This personal resume can
be attached with the school's one.
